Every state DMV is different. In the New York City area it is pretty simple. Fill out about a dozen forms from the DMV and hope you got all the forms you need. Provide at least eight notarized affidavits from all prior owner's dating back 50 years. Next provide six pieces of identification all with pictures taken within the last three years. Make an appointment with the DMV so that the wait will only be about three hours and lastly, hope and pray you get a DMV agent that actually knows what they are doing and really care about your problem.
