This most likely can be confirem with more info, i.e.-like is equipment 'original'; have you had an oil leak/drip/running from bottom of bell housing cotter hole; do you have grease 'zerk' on trans shaft collar..and othergreased it ?
If you have NEw equipment/rebuilt lately...adjustment of linkage and/or pressure plate out of adj specs.
