The title needs to be with the car and everything is checked by US customs before the car is loaded for shipment. A car with no title can never been registered in destination country - it can be shipped but with another code (parts, parts car, ...)
At the port of destination the car and paperwork will be checked again by customs. trhen the car needs to pas the technical inspection. If something is not correct you are in trouble.
(I speak for Belgium here - I imported some 55-60 cars from USA during the past 25 years - other countries may use different rules)
