Default Oversize cam gear?

When I see a cam gear described as .003, .005 etc, what part of it is oversize? Or undersize?

I saw one about 20 years ago that had a plus sign ahead of the .003. (+.003)
It was .003 over on the pitch diameter, not the OD.

I believe it's the thickness of the teeth, to compensate for a worn crank gear. The crank gear has half as many teeth as the cam gear, so will wear faster but the cam gear is easier to change in the vehicle.

If it was a - dimension it probably would be for when the mains are line bored and the crank is closer to the cam.
