I have taken a rubber mallet or rubber hammer and just hit it on one side then the other then stright on in those three areas and eventually it will work it's way out.

No damage.

A mixture of the both, lubricant around the base and some gentle taps from side to side while pulling upwards.

If you look in one of the "How to Restore Your Model A" there is a tool my dad sent in. It's fairly simple to make but he just listed the items and didn't put any pictures in it. I don't think it got a whole lot of attention, but is a great tool and doesn't damage the filler tube. I have his listing and also have two of them, which I plan to make a few up so I can make them available. I wish he'd put a picture of it as a picture is worth letting everyone see what it really looks like and works. It's like a dent puller on a slide and it fits around the filler tube and you just give it a couple of pulls and it comes right out with no damage. Private message me and I will look up the details and send some pictures.
