Check with a locksmith store in you area who has been in business a long time. It's probably a HURD key blank. I had one made for my 40 Ford a few weeks ago.
The replacement blank is not exactly shaped like the original HURD as it has a round top.
The blank # on the new key blank is 1125 H6
The numbers FK852 are the punch code. Locksmith can make a new key from those codes. Read more here:

Look here and you will see it's the replacement blank for HURD keys. Cost about $6.50 per key. Of course, you could buy the blank separately but, it will probably still cost you the FULL price of having one made.
