Check with a locksmith store in you area who has been in business a long time. It's probably a HURD key blank. I had one made for my 40 Ford a few weeks ago.
The replacement blank is not exactly shaped like the original HURD as it has a round top. The blank # on the new key blank is 1125 H6 The numbers FK852 are the punch code. Cost about $6.50 per key.
