Looks like the shaft was never checked for in and out movement upon installation. Due to wear in the head where the shaft rides a original spec shaft will move. The spec was around .006/.010" clearance if I recall. Today most use the shaft locking collar to keep it from excess movement. If yours has that collar, then maybe it came loose and just needs tightening(if not damaged). The 1/8" likely will not hit the radiator but the danger is supposedly the harmonic vibration could snap the shaft driving it into the radiator. I have seen a quarter inch plus!
