That's a 1955 (second series) through 1959 GM truck cylinder. Probably 1-1/8" bore, used in 1/2 and 3/4 ton Chevy and GMC trucks.

In the GM chassis, the pedal pivots in the fork on the cylinder, with no added hardware.
