In looking at the opening page, "FORWARD", it says,
"Compiled by BEN STAUB",
"Published by the Dayton Carrier Co., 6161 Rip Rap Road, Dayton, Ohio 45424"
"Ben Staub 1974"

Yes, I had a copy once, very nice, printed on glossy paper. Now I cannot find it, cannot imagine what happened to it.
