This is a box-type heater I bought twenty-plus years ago from J. C. Whitney for my Model A but was never installed. It looks to be the same one you are describing. It did not come with heater hose or shut off valve. It is designed so that a defroster could be hooked up with a bit of duct hose and other items. The fan is two speed and it came with the switch. I used it in my '32 and it does a good job.
Oh yeah, I paid about $70 for it back then.
