I have also pulled and rebuilt the driver.

Slot deep enough? Detritus in the driver from the sides of the hole thru the head, lightning

If you replaced the shaft the new one may need to be fitted. A shaved head, or decked block comes into play. And a standard length shaft(s) may now be too long.
