The photo shows two different hood formers, the 1915-16 and the 1914 and earlier style in that order shown. Both styles are for the brass radiator cars, the 15/16 was used on the sloped cowl cars and the <14 on the flat dash.

I asked to help narrow down your search. The 11-14 style could be attached directly to you firewall and can be bought new, the 15-16 bolt over a wood dash/firewall that would be then attached to your firewall. The 15-16 are not real common and are not reproduced.
